Abstract – Executive Summary
It took us over five (5) years to arrive at this format.  This report is centered about MSG’s healthcare provider Brand New Day (Universal Care) HMO.  Prior to being served by Brand New Day (BND) we were served and supported by Kaiser So CAL, the Orange County Health Care Agency, and HealthNet to name a few.  Now it amazes us here at MSG how large our BND Network has become.  This document charts our External Quality Review efforts about BND.  We call it our Annual Christmas Report Card or CRC.  Our 2017 CRC is framed about our prior builds.  BND as rated by the Centers for Medicare & Medicaid Services (CMS, 2017) as a 3.0 Stars program with 5.0 being the best.  This report contains some odd twenty-five (25) tables.  The tables include various dimensions of the BND Consumer Experience.  The theme for the tables is “Rate Your…”  Our Rate Your Lab (RYL) material is presented as a standalone.  We left our “Rate Your Attorney” for being too Out Of Scope.  Overall we here at MSG score BND higher on average than the 3.0 Stars awarded by CMS.  Last, we offer up a bit of our Lived Experience or LiveX as needed (PRN).  We are about to graduate to a new Behavioral Health Doctor.  He is briefly addressed in this report.  We have high hopes for this any new partnerships we mettle out in 2018.  Wow! 2018 can you believe it! :o)

Method – Brief
In the process of developing our Christmas Report Card (CRC) for Health and Human Service (HHS) practitioners we created several assessment tools.  One of these is our Vitals-Plus Assessment Instrument (VPAI).  We started with a foundation of the typical Rate Your Doctor provided on-line by “Vitals.com”.  We found that Vitals’ seven (7) -item instrument was inadequate to catch the HHS service experience.  Thus we extended the Vitals tool by sixteen (16) items we feel appends better resolution to the standard seven-item.  Our Mentalation(s) Solutions Group (MSG) Vitals-Plus segment has twenty-three items.  We will share our Theta Version with a link to follow.  We tested the Theta Version with one of our most important and error prone transactions – monthly scheduling.  Our methods behind the scenes are not as important at this time to share.  At this time we focus about results.  Below is a table of some of the filters we run for this our Annual Christmas Report Card (CRC) evaluation.

Built on Our Vitals-Plus Method (VPM)
For this report we use our Quick Score Method (QSM).  As we address an individual or agency we perform mental math.  Since we have performed so many rigorous Vital-Plus evaluations we memorized the questions (a filter) and ran the experiment (agency or individual) against our mental map.  In general, for us all test samples scored better with our QSM than by full blown granular assessments.  What follows are MSG’s CRC marks across many people and multiple agencies.  Enjoy!

Scoring – Stars, Grade Points, Grades, Binary
We here at MSG concentrate about three (3) quantitative scoring methods.  First, Stars scores with 5.0 Stars being best.  A Stars system is used by the Centers for Medicare & Medicaid Services (CMS) when scoring providers such as Kaiser.  Stars are also the system online rating services such as Vitals uses for rating healthcare professionals.  Second, we use the Grade Point system common for an educational setting.  A Grade Point of 4.0 (and sometimes 4.5) indicates the highest performance.  We didn’t include any of our in house assessment tool results/scoring yet we more often than not MSG scores applying a binary interval (0.0 = No and 1.0 = Yes).

2016 - Results Up Front (RUF) - Feature
As you will see we featured doctor TQN (Dermatologist) in our results up front section.  This Christmas Report Card (CRC) idea was born to share our gratitude with our highest performing doctor.  History - We first shared with doctor TQN for Christmas 2014 via a hand delivered Christmas Report Card.  One of TQN’s strongest features was and is providing for Informed Consent.  In addition, he is a really nice person with very good bedside manner.

You will see that rather than discuss much this year we focus on delivering the data.  Therefore RUF does not apply very much for 2017.  We will continue to hand deliver course-cards to a few of our highest performers such as doctor TQN.

Lived Experience (MSG-LiveX)
We here at MSG deliver our Brand New Day centered reports probono.  Our primary reward is that we get to share a bit of Lived Experience (MSG-LiveX).  Since we chartered our first Blogger Post back on September 19, 2012 we have found that we are including less and less LiveX.  Our LiveX publications draw more readers.

MSG’s Brief - Doctor Hanley (PCP)
Basically, our patient-doctor relationship with Doctor Hanley (JFH) began in 1961 when we were two (2) years of age.  We do not remember our first appointment yet when we asked him some fifteen years later JFH stated his medical records indicated it was for a head bump.  The first time we remember seeing a doctor other than JFH in the role of Primary Care Physician (PCP) was at UC Davis circa 1983.  We had contracted Pneumonia trying to adjust to riding our bike in: Rain, Sleet, Cold, Wind and Darkness (and more cold).  From our first visit onward, doctor Hanley saw us on average more than once per year.  We attribute our being able to type with all ten digits to his care.  He repaired both our left thumb and left pinky finger.  We also attribute being a live, period, to his care.  What seemed like every year we suffered from serious nose and throat infections.

At Risk
Yearly, we were at an elevated risk of dying.  Our risk was contained due to JFH’s antibiotic and peripheral care selections.  He was very smart to rotate antibiotics and peripheral treatments.  I remember when he moved from Benadryl [SPELLED] to Chlor-Trimeton Oral [SPELLED].  Chlor-Trimeton much helped us out.  We have one last note.  Our aunt (JMB) alive and aged 92 (B 1925), a cigarette smoker, completely refutes second hand smoke as a risk factor.  We grew up 1959-1964 immersed in smoke filled air.  One last disclosure:  We were injured (1963-1970) by other people so badly that we routinely required Doctor Hanley to mend us back together in his role as a surgeon.  JFH was weak when it came to addressing causalities about our health.

LiveX - Original Sin
Through applying MSG’s Promising Practice – Structural Temporal Reconciliation (MSG-STR) - we have reworked (processed) the times in life we have sinned.  Most if not ALL the time we come to the conclusion that our Original Sin was at age four (4) we smoked one of our Aunt’s cigarettes.  It was very early in the morning.  Our Uncle, my Aunt’s husband, had left for work at Sears and Roebuck [SPELLED] Los Angeles before six (6) AM.  We got up, went to the kitchen, found my Aunt’s pack of cigarettes, and lit one cigarette using the gas range.  This gas range technique we learned from mimicking our elder role models.  Before we could finish our first cigarette Aunt Joyce appeared asking Keith Edward are you smoking cigarettes.  Our response is: No!  Walla Original Sin!

Now we make a transition from text to our tables of findings (results) smoothly and without interruption.  We start with our work progress list that also acts as a table of contents.  We address some twenty facets for Satisfaction.  Our primary focus is our current health provider Brand New Day (BND).

Even though we manually deliver a few reports (course cards) by hand we rely on Blogging with Blogger to reach greater numbers of people that may benefit from our findings.

PEV
Our Performance Earned Value (PEV) concept originates back to April 26, 2011 when we were working with the OCHCA Data Outcomes Advisory Committee.  Since we were volunteering instead of merit increases we requested quantitative assessments or PEVs for our performance and contributions.  For example, we put in a good deal of time creating a Resilience Assessment Tool.  Paid county staff created their own Resilience Assessment Tool.  We ran them in tandem and they yielded similar scores about the same subject(s).  We saw another application for PEV also.  Many of the OCHCA programs indicated that they were using Best Practices.  Even though a strategy or process was deemed a Best Practice it didn’t make sense to declare it a Best Practice until it was proven about the new application.  Thus Best Practices really are not best until they achieve promising quantified PEVs.  We classify our Christmas Report Card as a Promising Practice or a PeePee or Double P.  The way we determine if this edition of our CRC is approaching best is by how many Page Views it earns on Blogger.  One hundred viewers would be Satisfactory for us.  Oh! We almost forgot – If Brand New Day makes good on our invoice that would also indicate additional and value added about Performance Earned Value (PEV).

FYI – Feature Doctor Clark - Preventative Rabies Vaccination
We addressed our family doctor John F Hanley in the Section of Honor.  Here we will briefly mention a doctor we crossed paths with in 1988.  We had been accepted to the UC Davis School of Veterinary Medicine.  Our cohort was some odd 115 students.  Before school started for fall quarter of 1988 we were instructed to show up at the UC Davis – Cowell [SPELLED] Student Health Center.  Because Vet Students' are at risk for contracting Rabies we were given a prophylactic Rabies Vaccine.  People really do not understand this method for human vaccination as compared to a remedial approach.  Keeping it short as possible, during the summer of 1988 we would have been considered highly functional.  Six months after the vaccine our functionality tanked.  We estimate we lost 75 points out of 100 about our Global Assessment of Functioning (GAF).  When we visited Doctor Clark the doctor that vaccinated us he gave us Tylenol for our headache.  What a mess!  We have said it before and will say it again.  As we keep pondering the Rabies Driven Sequela or Sequelae [SPELLED] our solution becomes more elegant or simplified.  FYI – Sequela [SPELLED] might be described as a condition that is the consequence of a previous disease or injury.  We will share the latest incarnation of our Vet Med Rabies Story hopefully soon.  What occurred to us is very rare.  Most in our support structure including Doctor Clark and Associate Dean Hansen at the time were caught off guard.

Progress Not Perfection – Error Reduction – Context Rimal B Bera MD
We don’t expect perfection our of our Behavioral Health (BH) doctors.  We do expect progress as indicated by PEVs using quantifiable results.  With our strategy we went nearly completely subjective this time around.  In prior reports we founded our reports on more objective data.  We want our next Behavioral Health doctor RB Bera (RBB) MD to be our best BH doctor ever.  We wish to inform him to the best of our abilities.  Repeatedly we have found evidence that our BH Specialists have treated us inappropriately.  We are changing doctors after we and Doctor Lee got it right for more than four (4) years.  Change poses a risk that Doctor Bera may wish to experiment with us.  Experimentation in this case would be attempting to fix us with new medication ignoring the fact that Clozapine works rather well.  In the future it is going to be up to the doctors to share their error rates.  Malpractice would not have a foot to stand on if it were not for MD generated errors.

Trends
There are two ways for a provider to earn a Neg (Negative) Trend.  One: We see them now and their performance has decreased quantitatively.  The other: We saw them in the past.  While reworking a particular Crime Scene we become enlightened as to how they were worse than we perceived earlier.  A down side of earning 5.0 Stars is that the trend may be stuck with Flat.  So Flat can be a good thing meaning doctor sustains their prior PEV or PEVs.

It wasn’t until we engaged about the local and state implementation of the Mental Health Services Act (MHSA) that we began to understand Directors.  To keep it short, Director Hale has been and is our HHS Director Control.  It has been eight (8) or so years since we risked talking to her in person.  Will we did so she immediately referred us away (Zager Principle, 2012).  We score her highly yet she will probably always remain our antagonist.

About two (2) or more years ago BDN Director Meier just up and disappeared.  She didn’t graduate before with us lingered a positive impression.  Director Emamian took her place.  We call Director Emamian the “Ghost in the Machine”.  She has never talked one-on-one with us.  We ask other members and comrades about her and they know little or nothing.  We did a little online searching to find a Curriculum Vitae (CV) or Resume comparable to Director Hale’s Resume (more a CV).

The title is a phrase (see ghost in the machine) coined by the Oxford philosopher Gilbert Ryle to describe the Cartesian dualist account of the mind–body relationship. BND Director S Emamian – Dualism
The dualism that we see in Director Emamian is that she appears to be present in body yet we do not see much of her mind.  Another Ghost aspect is: Since a mind has always been one way does not mean it cannot improve.  At last check Director Emamian is about sixty (60) years old.  We recommend she derives a 3-year Action Plan for self-reflection, accountability and self-improvement.  Of course the plan would include Quantitative Outcome Measures (Quant-OM).  We used to write five (5) year Action Plans for ourselves.  We based the timeframe on Stalin’s Five Year Plans.  Now we are older so we adjust a reasonable timeframe about three (3) years.

Value of Records in the Hands of a Consumer
We are in the middle of the Electronic Health Record Movement.  The various E.H.R. vendors include the value of Electronic Health Records in their sales pitches.  Part of the E.H.R. movement includes meeting Meaningful Use (MU) Criteria for providing patient access to records via Patient Portals. A selling point for patients accessing their records is: They may better team with their professionals about securing appropriate care and Better Outcomes.  Professionally penned records can save the consumer time when submitting mandated reports such as Disability Update Reports (DUR).

Do Overs – Universal Satisfaction (AVEY, 2017)
Unfortunately we have a minimum of one (1) appointment per month.  This favors chance for a scheduling error.  Error in excess of our other BND related transactions.  Another complication is that for the year 2017 the schedulers made some well documented errors.  Before Brand New Day and Central City Community Health Center we really never faced issues about scheduling.  For 2018 we are hoping to retire our Vitals-Plus high frequency monitoring in favor of CCCHC bringing back their Satisfaction Assessment.  We will not be-linger the issue: CCCHC is positioned to implement E-Satisfaction Survey Systems and Protocols.  The Consumer and Provider Satisfaction Movements are not going away soon.

Transparency Issue
When we research for our MSG reports we find it incredibly laborious to find out the first and last name of people in the network.  Thus for our CCCHC schedulers we settle on first names.  It took us about one (1) year to collect up the Front Desk Fearsome Four: Bethany, Beatrice, Gabby, and Jocelyn.

Value the Accounts
One of the founding principles in Customer Service is that you value your accounts.  A second principle would be that a Rep entertains some level of situational awareness about their accounts.  After dozens of transactions with Brand New Day their Telephone Reps are on the Dark Side of the Moon.  They do what they are paid for and account awareness is not one of them. Here is a Customer Service example: About ten years ago we moved from Irvine-CA to Anaheim-CA.  In doing so we shopped for a new car mechanic.  We found and decided on “Steve” the Service Manager at Firestone on Lincoln.  After our first transaction he always knew us and the car we drove and briefly our service history.  Even Hewlett-Packard knows our individual needs better that BND Customer Service.

Delta Dental – Set Backs
Brand New Day’s 2017 Contractor for dental is Delta Dental USA.  For most of 2017 we have been serviced by SmileWide Dental.  In October we receive a letter that beginning November 1, 2017 we are reassigned.  With SmileWide we are in the middle of a treatment plan.  Also before SmileWide we were assigned to a problematic Dentist (Saeed, Maisun DDS).  We called Delta Dental Customer Service knowing it is really hard to get a real person.  Our first transaction with Delta Dental we needed a Fee Schedule.  We had to file a formal Grievance with BND to get one.  Knowing the trouble to get a live Delta Dental Rep we hacked the dial pad options pushing numbers that are not in the choices.

Delta Dental - Getting A Helpful Real Person
We get a real person who tells us that we have to go online and research a new in network “Facility” ourselves.  We find the required Facility Code.  Call back and they promise us a Reassignment Letter by mail.  We have been waiting a month and we still are not reassigned.  What some of our comrades do is just not go to the dentist.  Some of our BND friends and comrades have rotting mouths.  Dental care is linked with some fairly costly systemic diseases such and heart disease.  Actually more of them suffer COPD, Medication Related Diabetes, and Medication Related Kidney problems more that heart disease.  About these ailments one cannot completely rule out dental.  We can’t react to everything so we are putting off fixing the assignment issue until after Thanksgiving (Today is November 19, 2017).
