We
pen and publish our Digital Appointments for numerous reasons. One: That is what ProSumers do! Keith “Buster” Torkelson is a consumer of
Behavioral Health Services and the principal with Mentalation Solutions Group
(MSG). He is also what we call a
pro-active consumer or a ProSumer. Next:
We apply our digital appointment for promoting some of our other
interests. Blogger powered by Google has
helped both Buster and MSG get vocal and remain vocal on behalf or our stakeholders. Voice, literacy and sharing have proven their
therapuetic value for MSGS’s principal Keith “Buster” Torkelson MS. Buster has not been in the hosptial since he
began Blogging. Our digial appointments
offer us a vehicle to Promote other MSG interests such our BedBusters Housing
Fix Bundler. Here in this report as well
as the shared version online we futher our investigations into the Rent A Share
Room (RASR) market and marketplace. We
do this because it helps Buster with self-stigma while in the process of
guarded disclosure. We also do it
because Buster has some great experiences to share about BedKeeping for the
disadvantaged in our communities.
Housing
the Disadvantaged
Due
to the fact that so many of Buster’s family and friends are successful they
really do not comprehend the RASR environment.
All five of his living siblings are married and home owners. In the last 20 years none of them have lived
alone or rented a share room where they sleep with in the same room with
another they do not love. With Buster we
her at MSG have been watching a videos on You Tube. In particular the videos feature museum
curator Ryan S. describing life on a Battleship. Buster’s dad saw combat from 1943-1945 on the
Battleship BB45 USS Colorado. Recently Ryan
addressed the topic of berthing.
Aparently a berth is “a fixed bed or bunk on a ship, train, or other
means of trasport”. He get to the topic “Berthing”. Those young sailors and marines sure knew was
living in a shared room was all about.
It is hard to persude those with little lived experience in a RASR that
it isn’t all that abnormal. For support
with delivering GreatBeds to Behavioral Health Consumers (BHCs) we will be
asking $1,000,000.00 per year to deliver on BedBusters Bundle. BedBusters is a fix oriented solution
addressing the unmet needs for the disadvantaged including the homeless.
Summary
of Ask for Help – Problem Solving
This report goes in tandem with our monthy face-to-face (F2F) with Buster’s new an improved Brand New Day (BND) helper. BND is Buster’s carrier for his health insurance. His helper identifies for his title and role as a Behavioral Health Care Manager (BHCM). Their next meeting should be May 11 (TU), 2021. At Buster’s last meeting in April 2021 Buster asked his new BHCM for help with a few things. The first obtaining mandated doctor’s approval for his Over-the-counter (OTC) smoking cessation materials to be paid for (subsidized) by BND. Another issue that Buster asked for help solving is getting paid his BND (subsidized) Reward for completing his Annual Wellness Visit (AWV). This task pays $50.00. We only get this once per year. As is usual the case we spend it on printer cartridges. We have an outstanding reward from 2020 for Buster’s Colonoscopy. This pays $25.00. We decided to abandon efforts for re-numeration and classify these monies as unrecoverable.
Housing Resources - Site Visits -
BedBusters (Beta)
In conjunction with our ongoing
major effort that we have designated as the Mentalation Solutions Group (MSG) BedBusters
Housing Fix Bundle we are making time to fine tune the product. BedBusters is designed to help fix housing
problems for the disadvantaged including the homeless. On behalf of stakeholders such as: The Health
Care Agency, Brand New Day, Bed Owners, families of consumers, etc. we are
reworking some of the extant resources.
We concentrate our efforts about a market share with housing market
Buster knows all too well. We call this
the Rent A Shared Room (RASR) market.
RASR is an alternate classification for Room & Board (R&B)
living. R&B is outdated,
stigmatizing and in many case incorrect.
Some define room and board as “lodging and food, typically forming part
of someone's wages or included in some other agreement”. Consumers doing work around the property or
for the (RASR) operation rarely get re-numeration such as a cut in rent or
actual payment. Our R&B (RASR)
resource lists go back as far as 2006.
They are out of date. In order to
update some 75-100 records we have made and remain impelled and at times are
driven to make phone calls. If the phone
number on a resource list is not in order we need to make site visits. This takes a lot of time and what we find is often
disappointing. In this report we include
a sample Housing Directory update. Ten
years ago the homeowner (bed-owner) charged around $500.00 per month for a
shared room. Now she wants $775.00. This doesn’t include food. We find very few consumers living with Supplemental
Security Income (SSI) or Social Security Disability Insurance (SSDI) having enjoyed
a $275.00 increase per month over the last ten years. We find a substantial disparity with
affordability here.

Engagement Summary – Time Spent
Psychiatric Advance Directives
(PADs) are rather new to us here at Mentalation Solutions Group (MSG) including
Buster. Buster has worked and delivered his
Advanced Health Care Directive (AHCD). His
health insurer Brand New Day (BND) has his 2012 AHCD on file. On April 1, 2021 Buster was invited to attend
an information/training session on PADs.
Mentalation Solutions Group (MSG) believes that advanced planning can
have a significant impact on both physical and mental health. In this report we discuss some of Buster’s
progress with PADs in general and his personal PAD. Since April 1, 2021 Buster has attended three
(3) PAD related engagements. Based on
the OCHCA PAD sales and execution timeline we will come in with Buster’s PAD
far before his OCHCA contemporaries will manage their first PADs. In addition will do it on a PAD per unit cost
basis our PAD for Buster will cost less money than an OCHCA driven PAD.
Summary Stigma
Due to the fact that the Orange
County Health Care Agency (OCHCA) is now investing about Stigma Reduction/Elimination
work, we here at Mentalation Solutions Group (MSG) are making a bit of time to
follow and possibly contribute to their Stigma related efforts. Over the years Buster has done pretty well
addressing his self-stigma and stigma related concerns. Later in this digital appointment report we
list some of MSG’s Stigma related objectives.
Our goal is to be able to help others in need. As objective #2 we promise to develop our
Stigma Impact Tool (SIT). Right now our
current version is SIT-Beta-20. During
April 2021 we scored Buster. His Stigma
Impact Score (SIS) is 60%. With our SIT higher scores are less favorable.
Summary – Interests &
Promotions Held Over (Spring 2021)
As is usual, when we begin a
quarter we get to decide what efforts to: Carry-over, abandon or kill, and what
we will do new. In this report we
summarize our interests for spring quarter 2021. Each quarter we make effort to review our
hardcopy and e-version of Keith “Buster” Torkelson’s MS CV or Curriculum
vitae. Since 2012 we have published at
least two versions. Back in 2012 we were
Doing Business As (DBA) Tork Reconstruction Company (TRC). Since 2014 we identify as Mentalation
Solutions Group (MSG). One of Keith
“Buster” Torkelson’s CVs is in a traditional format whereas another is a CV in
pictures. We need to go back and label
the pictures. Buster published his
Objective Record–driven Virtual CV back on Jan 6, 2012. We checked the number of page views on Apr
17, 2021. This CV has enjoyed 859 page
views. This makes us around Mentalation
Solutions Group (MSG) happy for him. If
only one in 100 would offer him a job it would mean he garnered eight (8) job
offers. In practice Buster really never
got turned down for a job he applied for.
Sometimes he landed jobs that he didn’t even apply for. Kathleen Murray his OCHCA supervisor at one
time turned him down when he wanted to be trained as a SuperUser for the
OCHCA’s Electronic Health Record system.
SPS-G8 = Social Pathology
Specialist Grade 8
Over the years Buster has earned
or been awarded many titles and roles. Even
as a kid some called him “The Little Professor”. It is too late for him to go
back to school and earn a PhD. He earned
Master’s degree out of the University of California at Davis in Pathology. His cohort was five persons strong. The other four were post-docs. Buster was the chair doctor Dungworth’s
ace-in-the-hole pathologist. At one time
Doctor D told Buster’s mom in person that Buster is both “eclectic” and
“esoteric”. On and off we here at
Mentalation Solutions Group (MSG) tapped Buster’s knowledge of Pathology in
various jobs and in several projects. We
are trying to get someone in the proper position to value his lived experience
in Sociology. We around MSG are secure
in our belief that Buster has earned his: SPS-G8 = Social Pathology Specialist
Grade 8. Last time he was graded in
industry was back in 1996 by the US Department of Agriculture. He graduated a GS-5. Not great yet pretty good for one (1) year of
service.
Corona Vaccine Status (4/2/21
& 4/29/21 @ Rite Aid – Moderna)
Let’s jump in Loki’s Time Machine
(LTM) back to January 2020 powered by the Tesseract which in turn was charged by
the Space Stone. We are back to January
2020. With respect to pulmonary health
Buster’s biggest concern was smoking and how to avoid needing annual antibiotics
for respiratory infections. Indeed in
February (2020) he found it necessary to leverage Brand New Day’s (BND’s)
telehealth benefit Teladoc to get a regimen of antibiotics. Then along comes the emergence of Corona
(COVID-19). Buster and his associates in
and out of the home initiate precautionary adjustments to stay the effects of
Corona (COVID-19). Buster as well as
many he came in close physical proximity (space sharing) stepped hygiene up
many notches. Then Wow! It occurred to us here at MSG that our
measures may help mitigate Buster’s annual lung infections. It did!
For the first time in a while during a cold season (November – March
2020/21) Buster did not need antibiotics.
He is now over a Corona Hump and met a milestone having had both his
COVID-19 Vaccines out of the way. We
have our documentation ready in case BND and/or CCCHC needs it. If so this would be an example of a Health
Information Transfer (HIT).
CCCHC Solicited PCP Evaluations
Doctor Comparative Score Summary
Central City Community Health
Center (CCCHC) is Buster’s clinic. To
best of our ability we here around Mentalation Solutions Group (MSG) believe
the Brand New day Buster’s insurance for health contracts with CCCHC. Near the end of last year and again beginning
this year CCCHC formally asked Buster to rate one of their doctors. The doctor in question is Buster’s Primary
Care Physician. Buster has been rating
is PCP for many years now. Whenever we
do a rating we try to find something or someone of comparative value. We selected Buster’s CCCHC related Behavioral
Health Doctor (BHD). In the 2020 rating
we gave Buster’s PCP 65.1% for Overall Practice Proficiency (OOP). His BHD earned himself an OOP of 59.7%. One of our favorite control values is CMS’s
Annual health insurance plan Stars Score.
For 2020 CMS awarded BND 3.5 Stars out of 5.0 being best. For 2020 we give Buster’s PCP 3.3 Star
Equivalents and his BHD 3.0 Star Equivalents.
For the 2021 evaluation both had improved a bit. PCP OOP = 79.2% = 4.0 SEs & BHD OOP =
68.8% = 3.4 SEs. We used our own
assessment tool derived from CCCHC’s tool for scoring both. We would appreciate it CCCHC sent us back
their analysis of their version so we could confirm our findings.
Summary of Time Spent and Due
Diligence
With Brand New Day HMO (BND) Buster
gets a whole bunch of health care. We
call any provider serving Buster in the name of BND: An in Network
practitioner. Apparently, BND contracts
with Central City Community Health Center (CCCHC). CCCHC has an association with Buster’s
Primary Care Physician (PCP). Since CCCHC
formally asked Buster to Rate his PCP we decide to briefly address two closely and
intertwined related features of practice: Time Spent and Due Diligence. Here is our ranking: Primary Care Physician =
Behavioral Health Doctor <<< Ophthalmologist < Dentist. Buster’s BHD also associates with CCCHC. We feel that Buster’s BHD should step it up
using our BND Network Dentist as a model for professional practicing.
Matrix – Characterizing Bullies
and Bullying
We feel that situational
awareness is import for Buster’s helpers.
Situational awareness is one of the driving forces behind our Digital
Appointment Product (MSG-DAP). Next to non-habit
forming medications’ that support SLEEP, we feel the bed and where the bed is
very high ranking with consumer health and welfare. In this report we here at MSG decided to
include a little material about roommates and housemates that Bully. Interrupting people in their SLEEP may be a
case of Bullying. Today is May 2, 2021
and Buster is very happy that his Rent A Shared Room (RASR) Roommate has let
him get in 3 nights in a row of uninterrupted rejuvenating SLEEP. We have both reports addressing out
SleepAbility product in the works and already published. Using a search engine of your choice execute

We just did and came up with about five (5) on target hits.
Assignment – Brand New Day
Ongoing - Quality of Life Assignments
Back in 2012 when Buster signed
on with Brand New Day (BND) the some odd twenty (2) page intake package included
a 15 item Quality of Life (QOL-15) assessment.
Buster on and off enjoys taking assessments especially those yielding
numerical results. From 2012-2014 Keith
“Buster” Torkelson would meet with his BND helper Oswaldo Escalante. Oswaldo was a Substance Use Disorder
specialist. On average they met up two times
per month. About once every three (3)
months Buster would have his latest QOL scores ready to discuss. Since the moment both Buster and Oswaldo graduated
apart none of Buster’s new BND helpers leverage their QOL assessment. Yet, Buster being the pro-active consumer, ProSumer
(MSG), he is still periodically runs his BND QOL-15. Since 2012 his BND QOL-15 scores range from: 13%-92%. They also fluctuate up and down. One of the domains that impact Buster’s QOL
is SLEEP and his daily SleepAbility (MSG product). QOL is so important to Buster and the rest of
us around MSG that we dedicate considerable time each weekend just to work on
it. Saturday is QOL as related to
technology and Sunday is QOL evaluation and development.
Meaningful Compensation - Circa
Winter Quarter 2017
The last time Buster decided to
pull him up at the bootstraps and target working paid full-time was 2011. He fancied himself as an External Quality
Review Officer (EQRO). He had built an
association with Dr. Horner in OCHCA Quality Assurance. He probably could have obtained a nice in-house
letter of recommendation from his supervisor Kathleen Murray. He went back to school to re-adjust to the
daily intensity of working under the rules set by others. The college he chose was Cypress College in
California. The Board of Governor’s Fee
Waiver covered all of his tuition. He
completed two Health & Human Services course both with an “A” grade. Yet, problems at home had become very
scary. His SLEEP and rest were
impaired. And in January of 2012 began a
series of losses. Additively we rate
these 2012 losses as: Catastrophic. He
was fortunate to have Kathleen and her (MHSA) Technology Advisory Committee
(TAC) to come back to. Buster’s
association with the OCHCA peaked during winter quarter of 2017. Dr. Horner and others gifted him with a free
ride to the annual California Institute for Behavioral Health Solutions
Conference. We here at Mentalation
Solutions Group (MSG) are for the long-term grateful for them and their
generosity. This is definitely a great
example of Meaningful Compensation (MC) in the face of serious disability. We include a record of this gift later in

Brand New Day HMO Blog
Around here at Mentalation
Solutions Group (MSG) one of favorite things to do is run that stats on our
various Blogs. Buster started a few of
them as a product of his Tork Reconstruction Company (TRC) efforts. Now MSG owns all of his Blogs. For Blogging we use Google’s free platform
Blogger. For the most part Blogger is a
good utility. Currently when we publish
anything associated with Buster’s insurer Brand New Day (BND) we do so with our
dedicated BND Blog: Brand New Day HMO.
We are pleasantly surprised with our BND HMO number of “Viewers”. We chartered it back on May 27, 2017. Since that time as of April 29, 2021 Blogger
indicates we have enjoyed 17,034 Viewers.
Many of them include viewer hits identified as Russian. In a given weekly cycle sometime our Russian
viewers exceed our American (USA) viewers.

Now in the face of Corona
(COVID-19) people are a bit more informed about risks and risk management. Some of the masks on the market consider
their Protective Values (PV). Here we
deal with the PV of person in Buster’s Health & Human Services
Network. On April 26 (M), 2021 we went
back and scored Buster’s Network. We
captured and calculated our Network Protection Since Onset Score (NP-SOS). Buster’s health related network earned a
NP-SOS of 48.1%. In 2012 Buster again
faced an episode of Catastrophic Losses.
His OCHCA network was not very helpful.
Again the ones that stepped up to the plate were his family member’s. Many disabled people do not have family to
help them out. We feel it is important
to deliver supports in the community that really address consumer unmet needs. The gaps are very clear with respect to
housing and bed quality.
Onset 19880923-F: Hand Notes
Even before the summer of 1988,
Buster was considered by some in his UC Davis cohort to be an “Unusual”
person. In the spring he received his
letter of acceptance into UC Davis’ School of Veterinary Medicine (Class of
1992). Yet, on September 23 (F), 1988
his Behavioral Health challenges began to emerge in an ever progressive and
debilitating manner. On that Friday not
only did become very ill it was in front of his classmates. For Buster this was stigmatizing because “Unusual”
became “Different”. He knew something
was going very wrong which impelled him to immediately take hand notes in time
series form. We include a scan of these
notes later on. You can see the chaos in
his mind from the deteriorated quality of his writing. One day we are going scan and capture some of
the changes over time about how he handwrites.
Summary Persons’ of Interest
Back in 2009 Keith “Buster”
Torkelson MS familiarized himself with the Orange County Health Care Agency
(OCHCA). He was initiated with some of
their works when they paid for him to train as a Mental Health Worker
Paraprofessional. He completed the 160
hour training second in his class. Up
until 2012 Buster grew his OCHCA network.
In 2012 his OCHCA network number of persons peaked. Buster is now focused on applying his lived
experiences with consumer housing to help resolve housing issues for the disadvantaged
including the homeless. In delivering
our BedBusters Project Idea and product he will continue to build his
relationships with about a half dozen OCHCA Persons of Interest (POIs).

First Occurrence
The first occurrence of Wider Circle (WC) in our database is August 28, 2019. We were referred to Wider Circle by Keith “Buster” Torkelson’s insurer Brand New Day (BND). We were initiated over lunch at a restaurant paid for by Wider Circle. Afterwards things went downhill and Buster ended his partnership and contributions to WC. During lunch the WC Rep provided a Self-care Checklist. We began transforming the checklist and lost interest with WC in about one month. The checklist did come in handy. Yet, none of their groups and ventures has appeal for Buster.

Basically Buster’s WC association had some personal value. Buster’s helpers never ask about what goes on with WC. WC has no value to either Buster’s Primary Care Physician (PCP) or his Behavioral Health Doctor (BHD). We transformed Wider Circle’s Self-care Assessment in order to determine the value of this resource. We calculated the percentages of new strategies. Out of 22 items approximately 2 were new to us. A few items although we considered them in the past we needed reminding.
WC Promise: “New programs & services available to provide critical support and social connectedness to seniors and other vulnerable populations in isolation.” It would appear that Buster is not in “isolation”. We don’t recommend WC for vulnerable populations that are not in isolation. Buster still gets one or more calls and or messages from WC each month. They have been coming in as “TOLL FREE CALL”. We have discovered: It is not the number of people you associate with it is the quality of the few. We have no associates with Wider Circle.

Taking
the Bait
While
in the process of getting put back on the OCHCA MHSA Steering Committee Meeting’s
(SCM) distribution of materials Buster reached out to his long time friend and
OCHCA employee Karla Perez. Rather than
send him SCM materials she sent materials for an Psychiatric Advanced Directive
(PAD) information session. Since Buster processes
an activates rapidly we call the information session for him a training. For those in attendance it was to sort out
the OCHCA MHSA PAD centered Innovation Plan.
For Buster the assignment is to understand and deliver his Beta PAD
ASAP. As with many of Buster’s meetings
the PAD Training was instrumented via Zoom.
During the meeting Buster took notes.
For the OCHCA their PAD movement is in the development stage rather than
the delivery stage. Yet, Buster is a
product-driven type guy so he went online to track down a PAD form so he could
get started.
Resource
Gathering
We
here at Mentalation Solutions Group (MSG) found on behalf of Buster a good PAD form
out of and legal in the state of North Carolina (NC). PADs’ differ from Wellness Recovery Action
Plans (WRAPs) in that the former is legal document. We finally tracked down a California PAD yet
we find it is not as user friendly as the North Carolina form. For now Buster will continue using our
transformation of the NC Form. Buster
intends to ask his Brand New Day ((BND) helper during their next Face-to-face
for a California PAD form in both hard and digital formats. Each time Buster meets with his BND helper we
will give a PAD update until we complete Buster’s and get it put in some of the
right place(s). The company facititating
the OCHCA PAD movement is called Concepts Forward Consulting (CFC). We plan to submit some of our PAD work to our
designated CFC Rep. During our training
the instructers indicated that pushing their PAD notions through with private
health care agencies will be harder that with public health entities.
Standardization
It
looks like in may take Orange County who hopes to fund their PAD movement
locally more than a year to select their standard and legally valid PAD
format. The PAD has to be valid in the
state where the consumer resides. In the
meantime, we here at Mentalation Solutions Group (MSG) have been digging around
online to find some materials to set us and Buster in motion. We have learned a few lessons thus far. One is we recommend two formats may for
driving consumers and any others in need working their PADs. The first format might be temporary and
open-ended. Consumers can copy anything
they feel is important for their individualized care and treatment from other
sources such as: Casenotes, medical records, journalling and/or their Wellness
Recovery Action Plan (WRAP) if they have one.
The second format will be the actual legal document.

Years
ago at the local annual Meeting Of the Minds (MOMs) conference Buster was lucky
to meet up with Mary E Copeland Wellness Recovery Action Plan (WRAP) people. He discussed how big his latest version of
his WRAP had grown. He said the process
had not delivered substantial results.
MEC’s people said scrap it and start again. Buster has found that WRAPs are not taken
seriously about the in-patient setting. It
appears WRAPs are best used as an intermediary on the way to PADs. We find many benefits in the PAD process yet like
WRAPs they are likely to be overlooked in either the in-patient or in the
county jail settings. Paper copies may
best be kept with the consumer, consumer’s family, care-givers, etc. When something comes up as conflicting
between treatment and PAD it will probably have to be the consumer that points
in out. We have here another experiment
using live subjects.
PAD
Legal
PADs
may offer people determination in an ever indeterminant world. We recommend that 25%-33% of the PADs by word
count address housing issues and concerns.
For the amount of money “The OC” wishes to invest in their PAD movement
we see legal fees. The state of
California is going to have to decide on one and is often the case more
standard and approved PAD format/formats.
If they can’t find one they like then of course they will need to retain
a legal staff to push a legally valid form through as hopefully one-fits-all standard. In the meantime we here at Mentalation
Solutions Group (MSG) are all over it.
Buster has already started his temporary PAD (tPAD) in *.doc format, he
has shared the PAD notion with his Brand New Day helper, and has tracked down a
codified version that appears to be valid in the State of California. We here at MSG believe that PADs are so
important that Buster’s insurer Brand New Day (BND) should design and deliver 1
if not 2 dedicated PAD Groups per week.

Spent – PADs - Adequate
Since April 1st, 2021 (TH) Buster has collected up enough PAD information to plot his PAD related direction. Rather than wait a near inderminate amount of time he’s going to complete the rough draft of his PAD. Then he is going to try to sell it to a few stakeholders: Rimal B Bera MD, Clayton Lon Chau MD PhD, his new and improved Brand New Day (BND) helper, and two of his family members. He might also promote PADs at the MHSA funded Wellness Center West (Garden Grove). He already has the knowledge base to teach a Mentalation Solutions Group 50 minute PowerPoint mediated lesson: “Fast Tracking your individualized Psychiatric Advanced Directive (PAD). He will publish his standalone PAD well in advance of milestones set by the county and their current team of PAD promoters.

PAD Portion Addressing Suicide
and Last Wishes
We have reviewed several
Psychiatric Advanced Directives (PADs) forms.
They don’t appear to include sections on suicidality, last wishes, or
housing needs. We suggest including
sections addressing integral concerns and problems for the Serious and Mentally
Ill population such as: In case of suicide, in case of suicide attempt, this is
what I want near the end, this is the kind of SLEEP I need, and these are my
housing needs. They need to address LPS
conservatorship and representative payees.
We know many a consumer that doesn’t trust their representative
payees. We suggest: A while a student
section, in case of harm to self, and help with stigma.
Funding
At this time it would be and
unnecessary distraction to delve too deeply into Stigma and Stigma Free OC (SFOC)
funding. Briefly: CalMHSA meaning
California Mental Health Services Authority is a Joint Power Authority (JPA). Years ago CalMHSA was delegated Mental Health
Services Act (MHSA) Prevention & Early Intervention (PEI) monies to manage
three areas of Behavioral Health at a regional level. In the case of Orange County we opted to have
CalMHSA deliver in three PEI components: Suicide, School Behavioral Health and
Stigma Reduction/Elimination. It makes
sense to connect Stigma Free OC with CalMHSA for (SFOC) funding. With CalMHSA and proof of concept in these
three areas we here at Mentalation Solutions Group (MSG) have yet to be
impressed.

Dying Too Young (DTY) – On
CalMHSA’s Watch
We are not going to go off on
Busters’ lived-experience with individuals Dying Too Young (DTY). CalMHSA was to substantially remedy the
suicidal persons in California. When
Buster grew up in SoCal and later matured in Davis California there were only
1-2 people he knew that executed suicide per decade. Examples: His neighbor as a kid and his
mother’s brother’s wife. Since CalMHSA
has been working on suicide solutions Buster knows at least four people in less
than a decade whose deaths were confirmed as suicides. Tim C shot himself in the heard, Matt hung
himself, and Mark jumped off an overpass into oncoming traffic. Another was Devin P. With Devin we are
uncertain the nature of his suicide.
Some say it was overdose related.
It is not as if Buster is hanging with a greater fraction of high risk
people than he did: 1962-2010. Another friend’s
son, Ethan B committed suicide yet Buster only acquainted with his father.

Basically Keith “Buster”
Torkelson MS practices pathology. Yet,
rather than work up biological and medical samples he processes written
assessments to capture the pathology. On
average Buster evaluates one assessment tool per day. In due course Buster analyzes assessments
including calculating and delivering quantitative values. Buster deposits assessments and his findings
in our Mentalation Solutions Group (MSG) database (MSGBase). Other than his Master of Science (MS) degree
out of UC Davis Buster is a matured “Para” health worker specialist. Buster promotes accountability and
transparency in his role about External Quality Review (EQR). Buster performs most of his work pro-bono.
20210417-SAT: On Measurement
Buster continues to find that
measurement remains out of alignment within the field of Health and Human
Services (HHS). He is hoping for some august
convergence in metrics along a manner with what Australia has
accomplished. Based upon Buster’s
learning at health technology conferences the US is still more than a decade
behind Australia when it comes to assessment standardization in Behavioral
Health. Some other areas that interest
for Buster are: Teaching workshops for adults, speaking in the community,
sharing on his Blogs, helping with events, supporting about the office,
“sprucing” up projects, and delivering on special projects. As of April 16, 2021 Buster is currently
working on a Medication Management Lecture Series for teaching groups at his
insurer Brand New Day HMO. Finally,
Buster is focused part time this spring quarter on fixes to deliver excellent
housing for the disabled including the homeless. We here at Mentalation Solutions Group (MSG)
call our housing project BedBusters. I

Keith “Buster” Torkelson MS’s
schedule for spring quarter 2021 is as follows.
Make record, measure, and improve the quality of his Health Related
Engagements (MSG-HREs). HRE is another
Mentalation Solutions Group (MSG) product.
For spring quarter we here at MSG as well as Buster have planned: Succession
in his support structure including medical and family, housing graduation, addressing
HHS Ethics as prescribed by Gary Zager at Cypress College, continue profiling
work that includes profiling his Behavioral Health Doctor Dr. Rimal B Bera,
leveraging Buster’s considerable lived experience acquired from advisory work about
the OCHCA MHSA Technology Advisory Committee.
His technology work included: Appraisal of Patient Portals and Personal
Health Records (PHRs). We hope to better apply the material from his PHRs for generating
his Psychiatric Advanced Directive (PAD).
We are considering reviving his newsletter Accountability. To spring quest that we could use help with
are: Get our Blog viewership numbers up across are our Blogs; Perform site
visits with Rent A Shared Room environments.
And last and the most important sell our BedBusters housing fix project
idea. What follow is a table addressing

Torkelson, MS aka Buster rather skilled at presentation yet nowadays he rarely
speaks in public. Part of this has been due to Corona (COVID-19). Buster is a writer with dozens of publications
under his belt. As of today, May 16,
2021 (F), we here at Mentalation Solutions Group (MSG) estimate that Buster more
than 5000 pages in virtual print. In his
capacity he out publishes both his Primary Care Physician (PCP) and Behavioral
Health Doctor (BHD) combined. With our
work here at MSG we have improved the digital presence of both his current PCP
and BHD. Buster qualifies as a desktop
publisher and enjoys adding his own art and graphics to his as well as MSG’s
Blog postings. Buster’s hardware and
software computer skills are too numerous to count (TNTC). You can find them on his virtual CV.

MSG Cross Training
Unfortunately, Buster he is only
literate in English. Yet, literacy
appears to be one of Buster’s Protective Factors (PFs). Buster understands and speaks a bit of
Spanish just enough to partner with Latinas.
One might say Buster is also experienced resolving conflicts such as
family issues including divides. He has
re-united several families. Unfortunately
he has not re-united his own family.
Buster has a history in education and enjoys presenting, instructing and
teaching. He believes that Behavioral
Health Innovation (BHI) is still falling short in cross-applying (innovating)
promising practices from California’s Public K-12 system. Buster is an informal apprentice
blacksmith. Around Mentalation Solutions

Solutions Group (MSG) each Friday is Satisfaction Day. Keith “Buster” Torkelson MS has been at
Blogging since 2012. Checking Buster’s

Satisfaction Friday. We estimate that
MSG and Buster have more than 5000 pages in virtual print. Some of these virtual pages (entries)
constitute part of our Digital Preservation Effort or DPE. We have two plans for later. 1) Eat ice-cream for lunch and the other go
out together after work tonight. We have
no idea what EMe is thinking because he decided to sleep in this morning. He may show up for ice-cream. (ACA)

away from the home office and at our Deep Space 226 facility is limited. Even in the face of Corona (COVID-19-Plus) Keith
will get out for: Health Related Engagements (HREs), conferences, committee
meetings, and site visitations. Many
tasks cannot be completed using Zoom and the like. Before Corona Keith performed more site
visits than he does today. Gradually
more sites are opening up again. Through
his site visitations we collect “Intel” or Intelligence in Health & Human
Services including photographs we use in papers and presentations.

Target Audience
Obviously, a primary target
audience for this our Digital Appointment is those instrumental about Buster’s
Health – Education – Welfare (HEW). Public
Health raves about coordinated care. To
actually sustain coordination as a consumer migrates through their system would
require a “Stellar” care-coordinator.
After firing Buster’s last care-coordinator we here at Mentalation
Solutions Group (MSG) are giving our New and Improved (N&I) one a
shot. We definitely know he is new while
assuming he will perform on behalf of Buster “Stellar”. More or less this
report targets Buster’s N&I care-coordinator. His N&I care-coordinator calls himself a
Behavioral Health Care Manager (BHCM). In last month’s face-to-face (F2F) meeting
Buster asked for help on a few things.
We know as of today, April 28, 2021, that one hasn’t been
accomplished. For our next F2F with
Buster’s N&I resources we are without fanfare just list what we asked for
in one simple Memo. For the remainder of
our target audience we will address sections as needed (PRN). Even though Buster’s Behavioral Health Doctor
(BHD) indicates that Buster is acting as if a Housing Advocate his BHD doesn’t
have the time to gather knowledge including situational awareness about
Buster’s ongoing housing interests, pursuits, and concerns. Repeatedly in Buster’s health history Anxiety
and Sleep Disorders ebb and flow. At
times they put him in the hospital. His
current BHD is resting on the laurels and the experiments of doctors’
past. For Buster’s current BHD, we truly
can’t say that on Buster’s behalf that he has achieved something. Buster just turned 63 and now he very likely
is suffering symptoms of ADHD. Buster
knows several people who were treated for ADHD and they indicated that in the
long-term medication is not a viable solution.
The last thing Buster needs is a stimulant.
Role of Buster’s Behavioral
Health Doctor
Buster’s appointment with his Behavioral
Health Doctor (BHD) lasts on average less than 10 minutes. The frequency used to be every 3 months yet
his doctor favors every 4 months.
Buster’s BHD really can’t find or make time to listen to Buster. Therefore Buster and hasn’t addressed his
ADHD concerns. It would be too much work
for his BND. Around here we get to
prioritize and prune distilling out Buster needs that pertain to his BHD in a
given appointment.
On-line - More on Audiences
Now we feel it is important for
Buster to keep his BHD apprised of the Psychiatric Advanced Directives (PADs)
movement and portions of this report dealing with PADs. Yet, even if Buster submitted a draft version
of his PAD his BHD doesn’t have the capacity, time, or skills to review it
properly. There are several elements of
this report directed at Buster’s Primary Care Physician (PCP). Most importantly is Buster’s Corona
(COVID-19) vaccination status. As of
today May 3, 2021 buster has completed both shots at Rite Aid with Moderna tow
part Vaccine. There are parts of this
report that Buster’s PCP might best share with his clinical and clerical team
at Central City Community Health Clinic (CCCHC). Vaccine status is one of them. Back in 2012 when Buster signed on with
current insurer Brand New Day (BND) they demonstrated a high level of
coordination and collocation. We used to
just share our report materials with BND corporate via the grievance
process. Yes, indeed corporate could
benefit by reading this report. Late
last year we secured evidence that a person or persons around BHD had read one
or more of our reports that we posted on our Brand New Day HMO blog. This makes Buster happy!
Family Involvement
A few parts of this report are directed at Buster’s family. Examples would be: Corona vaccine status, blog stats, PADs, Quality of Life, Protective Value, and anything related to Advanced Planning. We hope that by summer of 2021 we can go without masks for family get-togethers. We have some advanced planning to do.

Practitioner Measurement
Let us just call everything in
Behavioral Health the BH System. Some key agencies in The BH System are Centers
for Medicare & Medicaid Services (CMS), Medicare, Centers for Disease
Control (CDC), and more locally the Orange County Health Care Agency (OCHCA)
and Los Angeles Department of Mental Health (LACDMH). We bet that in the near future LACDMH is
going to phase out the MH for the more proper Behavioral Health (BH). “The BH System” promotes and collects up data
left and right. One of the most straight
forward and elegant reports is the annual CMS Stars Scoring for health
insurance carriers. Buster’s insurer
Brand New Day for 2021 was award 3.5 CMS Stars out of 5.0 being best. BND contracts with Central City Community Health
Center (CCCHC) who appears to employ his Primary Care Physician (PCP). Last year near the end CCCHC asks Buster via
Email to rate their employee, Buster’s PCP, using their online assessment
utility. Buster followed through and
submitted a pretty good review. We are
waiting for feedback from Omar CCCHC’s CEO for our effort(s).
Patient Experience Measures
(PEMs)
During this last winter quarter CCCHC
(Central City) asked Buster a second time to review his PCP. Again, Buster followed through because he is
what we around here at Mentalation Solutions Group (MSG) refer to as ProSumer (MSG)
that is ProSuming (MSG). For the life of
us we thought we had vetted the Central City questions elsewhere. Central City indicated that we were taking a
Patient Experience Survey (PES). So queried
our databases for Patient Experience Survey or PES and found a few hits. For us and in our databases PES means Personal
Empowerment Scale (PES). Yet still we
knew we had seen the Central City line of questioning elsewhere. We performed
an old-school Window XP query. We found
Patient Experience Measures (PEMs). The
surveying that CCCHC performs as Patient Experience Survey is a misnomer for
Patient Experience Survey.
Lesson Learned
If a governing or control agency gives you an assessment do
not rename it!

Measures V Stars
How the Centers for Medicare & Medicaid Services (CMS) derive their Annual Stars Scores (ASS) still eludes us. We do know that it is fairly complicated. They contract with data processors just to address the incredible amount of information they receive each year. CMS’s method appears to consider Health Outcome Surveys (HOSs) and Patient Experience Measures from the CAHPS® Clinician & Group Surveys. CAHPS means: Consumer Assessment of Healthcare Providers and System (CAHPS). We have yet to find a CMS Stars Rating Formula (SRF). Since the scores from the survey we submit are not provided back to us we at present use our methodologies in order to capture the patient experience. Our Annual Christmas Report Card is one means for us to share our patient experience findings. Brand New Day (BND) for 2021 was awarded by CMS a Stars Score of 3.5. Most of their performers, contractor, etc. deliver 3.5 Star Performance. Yet we have discovered high performers even some near 5.0 Stars. This indicates that there are low performers around 1.0 Stars. One of our sources indicates that BND is a non-profit and non-profit have trouble getting good workers and contractors.

HITs and HIEs
It has been quite some time since
the OCHCA BHS went live with their first build of the Cerner Electronic Health
Record. In the process of “Going Live” they
apparently had satisfied adequate Meaningful Use Criteria (MUC). Some of the elements with Meaningful Use were
(are) Health Information Transfers (HITs) and Health Information Exchanges
(HIEs). As of 2021 we find that the
Brand New Day Network (BND-N) is still challenged about having the right digital
information in the right place at the right time. We were asked to perform a HIT by Buster’s
Brand New Day contracted Ophthalmology office.
We picked this Medical Record to share because it isn’t too revealing. At this time this record fits our “Guarded
Disclosure” criteria. Buster’s
Ophthalmology office is one of the most important providers for Buster in the
BND Network. They always earn Star
Equivalent (SE) score of 4.5 or better.
The down side it that a 4.5 for an in network office predicts for an
office operating at only 2.0 SEs.

We consulted with another doctor working with the National Institute of Health (NIH) about the Assessment of “Patient has type II pre-diabetes…”. She asked us for our A1c and concluded that Keith “Buster” Torkelson was not pre-diabetic. Unfortunatety this is a Medical Record Error (MEE) that may stick for quite some time is not hundreds of years.

When Buster first outlined his
initial Blogging strategy he determined that we did not wish to field
comments. So we wrote and published so
as not to generate feedback. Relatively
speaking to only have less than 100 comments since 2012 he and we have been
successful. We hope that by 2023 our
AnimaCules Blog will pass the 100,000 viewer mark and our Brand New Day HMO
Blog will exceed 25,000 viewers. In a different time and life Buster would

Psychiatrist
Unfortunately psychiatrists have
worked their way into a stigma hole. Many
people still feel uncomfortable indicating that they retain a
psychiatrist. There are other areas in
Behavioral Health that are climbing out a stigma hole. For example the state mandated Mental Health
Boards (MHBs) are reorganizing into Behavioral Health Advisory Boards (BHABs). Years ago Doctor Arnold P Deutsch (APD) predicted
that psychiatrist would work their way out of effectiveness. ADP indicated that the Primary Care
Physicians (PCPs) could spend time prescribing psychotropic medications.
Minimal Billing
When Buster saw APD it was on a cash basis for a therapeutic hour (50 minutes). Buster and APD made good use of the time spent. With Buster’s new and improved BHD he is lucky to get 10 minutes. At one appointment Buster told his BHD that it is sad about time spent and how his BHD runs his “practice”. Buster is billed for Medication Management. His current BHD really isn’t adept at Medication Management. Actually he has made some mistakes. More often than not his staff covers them up for him. Considering Buster doesn’t get invoices such as those APD or even Kaiser Permanente provided he can’t tell the level of Office Visit his BND is billing. This is a lack of Transparency and Accountability. We just found some indication that the minimal billing in psychiatry for an office visit in 30 minutes.

others. Buster is hoping that what his
generation learned can be leveraged into Better Outcomes for kids growing up

Group (MSG) have several formats that we use to evaluate doctors. Two of our FAVs are our Vitals Plus method
and our Boy Scouts of America Virtues Assessment (BSA-VA). We just had Buster’s oldest sister rate him
using the BSA-VA tool. She scored over
nearly the same as Buster scored himself at the beginning of spring quarter
2021. This is part of our reality
testing efforts. Dr. Deutsch introduced
Buster to Reality Testing back in the late 1980s. You can search “keith torkelson doctor on
call” to see some of our work that includes Dr. Deutsch. We just did and pinged five of our related
papers using Google to search the web (WWW).

from Keith “Buster” Torkelson lived experience in a RASR environment. SLEEP is in the service domain for both
Buster’s Primary Care Physician (PCP) and his Behavioral Health Doctor (BHD). In reality both only offer help tangentially. Too much Bullying in a given day generates
fatigue and worry. If it happens in the
middle of the night it may directly impair SLEEP. Buster has shared a room all in the list than
follows except: Wayne, Richard and Ted.
Hot of the presses!. A couple of
days a Wayne and Richard had conflict of a plug receptacle in their shared room. There was a push and shove. The outcome was that Wayne punched Richard
good in the face. This is important and
timely because Wayne is a repeat offender.
Now we have cause to share a photo of Wayne, Carl, and Reggie the kid. We have more evidence that Wayne has anger
issues and is an overt Bully. Wayne on
top at over 250 pounds breaks Jessie’s nose.
Jessie weighed in that day at about 150 pounds.

Quality of Life and Satisfaction Summary
On behalf of Keith “Buster”
Torkelson we here at Mentalation Solutions Group (MSG) began collecting Brand
New Day (BND) centered Quality of Life (QOL) stats starting back in 2012. We used the assessment tool provided by BND
for its’ consumers in the intake pack. MSG
finds it easier to impair your QOL than to build it. For Buster and many others
QOL ebbs and flows. Applying the BND
tool in time series from 2012 to the present Buster achieved a low of 13%
(2013) and a high of 92% (2018). Our
last run with the BND QOL assessment tool yielded for 20210131 (Sunday Real
Time) a BND QOL score of 85.6%. Most
people might say this is good enough and Buster should be satisfied. For the most part Buster is Satisfied with
his overall QOL for 2021 thus far. Yet
that 15% or so of loss is in a priority area: Peace, comfort, SLEEP, and SleepAbility about his current bed
(GoodBed). Corona also has reduced
Buster’s QOL as measured by (AMB) BND’s QOL instrument. The following tables summarize data about capturing in Buster’s time series using with
BND’s QOL survey.

When Buster signed on with the
Orange County Health Care Agency (OCHCA) back in 2009 Mark Refowitz was the
Deputy Director of Behavioral Health Services (BHS). Since Buster was a formal volunteer rather
than an on the clock employee he got to pick his bosses. The following were his bosses from high
frequency dealings to low: Kathleen Murray, Dr. Horner, Dr. Clayton Chau, Kate
Pavich, Mark Refowitz and Dr. Dorman. At
one point Buster would also consider Mark Refowitz a friend. Eventually Mark moved up to OCHCA Director
and Buster didn’t see him much in public anymore. Mark was succeeded by Mary Hale MS. From the beginning Mary was an antagonist for
Buster. When is it that an antagonist
can be your friend? ANS: When they are
correct and/or teach you an important lesson.
Attitude of Gratitude
Dr. Jeff Nagel appears to have
succeeded Mary as HCA BHS Director (HCA Deputy Director) in 2018. Yet, Buster had been learning from Dr. Nagel
well before 2018. For Mary, Buster, and
Dr. Nagel: Two key events would be: Somewhere around Thursday, September 18,
2014 Buster attended an OCHCA MHSA Innovations Component Training in Santa
Ana. Dr. Nagel was one of the
teachers. And, it would appear that
around March of 2018 Mary was asked to step aside over the unsolved homeless
issues in the OC. As we said: Dr. Nagel
stepped into the BHS Director slot. At
that time Buster had been following him for about four (4) years. During winter quarter of 2017 four OCHCA
executives approved to send Buster to the 17th Annual National
Behavioral Health Management Conf & Expo.
The reward was for $759.00. The
approval was since by three person with whom Buster acquainted: Horner, Refowitz, and Nagel. Since Dr. Horner and Mark Refowitz have
retired Buster can only sustain his gratitude through Dr. Nagel. We have been tailoring our BedBusters product
as if we were going to get an opportunity to sell it in person to Dr. Nagel.

2018 an End to Mary – Protective
Value
One time OCHCA BHS Director (HCA Deputy Director) Mary Hale is an antagonist with Buster. As compared to her contemporaries such as Dr. Chau and Dr. Dorman Buster found her hard to communicate with. Yet, Mary taught Buster that “Housing is a complex issue”. There are much more enjoyable pursuits in Public Health than Housing. An example would be consumer computing and technology for recovery. Yet, for many housing still remains an unmet need. Somewhere around Thursday September 18, 2014 Buster met Dr. Jeff Nagel. Dr. Nagel is user friendly. From 2008 till 2012 Buster built his OCHCA network. However, in times of crisis Buster found little Protective Value (PV) in his OCHCA network. He did find PV with some of the processes they taught him such as psychosocial assessment and treatment planning. He also found PV encouraging resilience to return to his OCHCA commitments ASAP. Last Monday (4/19/21) Buster attended the Zoom SCM slot from 1p-4p. At one point they discussed how to improve Marketing about OCHCA BHS Programs. We here at Mentalation Solutions Group (MSG) are in the same boat of how to achieve increased readership/viewership about our Blogs.
After being in the OCHCA BHS
distribution for more than three years Buster was dropped from the
distributions. From 2009 till 2012 he
was in the distribution for MHSA Steering Committee Meeting (SCM)
materials. About the time the county
ended the MHSA related Consumer Action Advisory Committee (CAAC) the SCM
materials stopped coming via e-mail. FAQ
– Are in person meetings unnecessary? Think
about it. Much of what we here at
Mentalation Solution Group (MSG) are doing this spring quarter is to submit our
really cool and happening county-wide house fix bundle. We call it BedBusters. Our preferred route for funding is securing OCHCA
MHSA Innovations monies. Last Monday’s
SCM participants spent just over two hours brainstorming. At 3 O'clock they addressed Innovations (INN)
budgeting. Good news is we finally know
the current INN projects. Bad news is
they have gone bigger with fewer.
Compared to some of the current multi-million dollar projects and budgets
our housing fixed would come in as a small project. We think the change to fewer projects per
cycle has to do with the Mental Health Services Oversight and Accountability
Commission (MHSOAC) turning down to many of the OCHCA plans. We don’t plan to put in much effort in
network building except when it comes to delivering BedBusters.

OCHCA MHSA Steering Committee
Meeting (SCM)
Last Monday April 19, 2021 Buster attended the mostly monthly Mental Health Service Act (MHSA) Steering Committee Meeting using the Zoom formula. Presenters, members, and persons in the community discussed: Improving [consumer] technology skills. The MHSA in Orange County is preparing to step up their investment in consumer technology both hardware and software (Help@Hand we think). Buster didn’t share yet what follows are some of Buster’s thoughts with regards to initializing and or upgrading consumer technology. Buster is very fortunate to live in a house were Broadband Wi-Fi is included in the rent. In his Rent A Shared Room (RASR) household there are eight (8) individuals other than Buster that could have access to the internet. Only one, Buster’s roommate uses the internet. Buster using his internet time educates himself. Whereas Buster mostly uses his point of access for productive endeavors his roommate uses it for what he calls watching TV. TV is app mediated services such as Pluto and Tubi. We feel that if consumers are offered free internet it would be prudent to guide them into a performance and productivity approach. We here at Mentalation Solutions Group promote Life-long-learning when offered the chance.
Brand New Day Computing
Once again, Buster signed on with his health insurer Brand New Day (BND) back in the summer of 2012. At that time BND had some resources that have long since been taken away. We guess they were unnecessary. Three of the resources that the members’ thought helped them were the free consumer phone, the consumer computers (x2), and free internet. Due to Corona BND has also removed work surfaces and access to care-coordinators in their offices. We guess these also were unnecessary. Buster made use of the computer resources in creating his BND driven Personal Service Plans (PSPs). Now in 2021, BND has no internet assessable for its’ consumers. Like this is way old-school huh.

Buster’s First OCHCA BHS Related Assignment
Buster graduated 2nd
in his class from the MHSA funded Workforce & Education Training (WET)
Consumer Training Program at the end of spring quarter 2009. During the summer 2009 he interviewed at the
MHSA Center of Excellence (COE) for a job working with the MHSA Innovations
Advisory Committee (IAC). Kate Pavich
the boss said: “The IAC is still a work in progress yet in the meantime we
would like you to participate in Technological Needs as the consumer Rep with
the MHSA Technology Advisory Committee (TAC). Buster’s first assignment from
Kate was become a member of the Santa Ana MHSA CSS funded Wellness Center and
bring back an evaluation for us here on the TAC. Buster selected the Horses Heal Program. The foundation of his evaluation was a brief
consumer satisfaction survey. Our
records in TRCBase indicate that he was ready to present to the TAC on December
15, 2009.

to Wellness Center Central (Orange) and observe its’ leadership post-Patti and
post-Corona operations. Patti was the
Wellness Center (Santa Ana-Orange) founding director. She retired and many report that has died.
On June 2, 2011 Buster was ready
to present on disparity and disproportionality.
During 2011 Buster performed a good deal of work as a Technology
Advisor. His primary projects centered for
consumers of Behavioral Health Services (BHSs) such as Computing resources in
our community, personal health records, patient portals, delivering kiosk
consumer computing to the HCA-BHS clinics, consumer resilience, consumer
empowerment, and solving consumer/family digital needs. At the end of 2011 the Santa Ana Wellness
Center was readying to move to their permanent location in Orange. Some Wellness Center employees were
apprehensive about the Move. Aside:
Moving can be very difficult if not impossible for some. Working with his friend and Wellness Center
employee, Denise C, Buster collected up the Intel including inside photographs of
the new facility and then co-presented to Wellness Center staff. Patty the Wellness Center director at the
time indicated that his presentation was helpful for her and her staff.

For documentation and digital preservation purposes we might convert some of our PowerPoints to PDF and publish them on one or more of our Blogs. Continue work with disparities in housing the disadvantaged including the homeless in Orange County and elsewhere.

Structured Temporal Reconciliation (MSG-STR)

When consumers are facing losses on a catastrophic scale they need people to step in and help. At Cypress College in Crisis Intervention class Mr. Zager the teacher taught us when a person or group exceeds their capacity to cope they may need an intervention. We here at Mentalation Solutions Group feel at the core of the intervention is providing GreatBeds (MSG) that facilitate: Resilience, healing, recovery and setting things right. They need a home in which they are guaranteed routine, regular and rejuvenating SLEEP. People in their network mattering most are those associated with stopping the fall and in person help deliver this allusive GreatBed. Now, we will not detail what makes up an effective Crisis Resolution Network (CRN). After-the-fact Buster’s HCA network was not very, if not at all, helpful for him in times of crisis circa 2012. For Buster his crisis had escalated far enough that he had high chance for accidental DEATH. Below we score some network features. One may notice that we offered no 1.00 = 5 Stars = 100% for any network area. It was Buster’s family involvement that helped the most. We give Buster’s family a Protective Value (PV) and intervention score of 90%.

Motivation – Family has mistaken
beliefs about Buster’s adherence to psychotropic medications. This is annoying and disappointing. When we get the time we will reprocess the
incidents in question. Since first
receiving psychotropic medications back in 1989 Buster has never had a day
without them in his system. In 2012
there was a Clozapine gap were a prescriber decide to take Buster off Clozapine
cold turkey as substitute is with Invega injectable. Without much ado Buster has been back on
Clozapine since fall quarter of 2012. This
is just one “Experiment” on a live human gone terribly wrong!

Vet School Post-Admit Criteria
Back during the summer of 1988
Keith “Buster” Torkelson was on top of his game. He was wrapping up the coursework and
research for his Master’s work at UC Davis.
During spring quarter he had received his letter from UCD Vet Med he was
accepted in the class of 1992 for professional work. During the summer he found replacements to
rent two of the rooms in his 3 bedroom apartment. One of the roommates, Jon Whitney, would have
declared Buster rather healthy. Buster
was still putting in long hours though.
Some might say he was hypo-manic yet it was hard to differentiate from
just plain old excitement. The acceptance
letter outlined the next steps for all students. On September 21st, 1988 (19880921-W)
the class of 1992 was indicated to show up in alphabetical order for some
tuning up at the student health center.
Since Buster is a “T” his group of 4 had their appointments in the 10am
slot. It was an exciting moment because
Buster got to meet some of the students he would associate with for the next
four years. One of the treatments that
day can be associated with some a series of life changing events. Without Informed Consent including a list of
adverse reactions to watch Buster paid $172.50 for Injections and Immunization
– Rabies Series (x3). This not a Rabies
Treatment after exposure rather it is Rabies Prevention and Prophylaxis. The first night and second night were
unremarkable. At some point Buster began
a hand written timeline which now can be used for our Structured Temporal
Reconciliation (STR).
Self-Stigma Onset
Approximately 48 hours after the
first dose of Rabies (prophylactic) Vaccine Buster experienced onset of
symptoms. We have discussed this
elsewhere so we will jump to the onset of self-stigma. On Friday September 23rd was
orientation day for the UC Davis School of Veterinary Medicine Class of 1992. Buster showed on time in the right place. Our notes indicate that whole class was to
first buy our books and syllabi. Next,
we boarded buses and went to have our photos taken. After photos, we were treated to a tour of
the Vet Med Live Animal Research Facility.
Just after we got of the buses Buster felt sick with headache, photo
sensitivity, and noise intolerance. . In addition, he was most likely
nauseated. After touring one house of
animals he felt he needed to lie down.
He was told he could cut the tour and orientation and go home and rest
up for the start of classes next week.
This was when self-stigma in the context of Vet School for Buster set in. He felt different and this would persist
until his final medical leave from Vet School in 1993. Buster asked around and only one other of his
classmate reported problematic symptoms after their Rabies Vaccine. Buster will always be different than his
classmates and friends that completed Veterinary School. We call this phenomena and incidence for him
acts of “The Big Fat Foot” and “Mr. Take Away”.

hindsight this event was worthy of investigation as a potential Vaccine Adverse
Event (VAE). Dr. Hansen the Dean of
Student Services said in conversation that Buster’s (Keith’s) Rabies Antibody
Titer after one dose indicated that he was having an amnestic response. For one of Buster’s Rabies reports we asked
the Student Health Center for Medical Records 1988-1989. They had been destroyed and now are lost to TIME.
Volunteer Work Network
Periodically the Health Care
Agency (HCA) shares and or publishes an agency organizational chart
(hierarchy). Buster picked one up around
2013 at the annual Meeting of the Minds (MOMs) Conference. The 2013 version outlined characters in the
HCA that delivered Behavioral Health Services (BHS). From 2009-2012 Buster was formal volunteer
for the HCA BHS division. Buster
associated and worked with/for about 5 of them.
His total number of direct associates about the HCA BHS was more than
100 individuals. Buster became very ill
2012. We here at Mentalation Solutions
Group (MSG) call this a period of catastrophic loss. When Buster resumed work about the community much
of his OCHCA BHS related network were unavailable. As a consequence of losing so many network
players we are working to rebuild around fewer stakeholders. As Buster often told Joan Z. his Most
Significant Other Ever (MSOE) “Less can be more”. Below is a table of associates about the HCA

Mentalation Solutions Group (MSG)
began selling our Housing Solutions Idea (HIS) back in the summer of 2018. At that time our idea was called Housing Advisory
Board (HAB). We met in person with Dr.
Nagel to present HAB for him. Then
Corona hits and our usual and customary was deemed unnecessary. Our HAB would have been organized and funded
as is the Mental Health Services Act (MHSA) the Steering Committee
Meeting. By fall of 2020 we had
incorporated our HAB notion into a larger series of fixes: The BedBusters
Bundle (MSG). We decided a good route to
deliver BedBusters would be via MHSA Innovations Component monies. As we were getting ready to submit we encountered
some unexpected hurdles. The MHSA INN submission
protocol has changed. Thus, for spring
quarter 2021 rather than moving through to the planning phase we are moving
retrograde back to submission. We need
to figure out who, when and how to submit.
We have attempted to figure things out via the dedicated MHSA INN
website. We have made several phone calls
including a few to the OCHCA BHS referral line OC LINKS. We made several phone calls to both the MHSA
or MHSA INN offices. No-one picks
up! Next Step: Action Item: Contact the
OCHCA MHSA Innovations Coordinator Sharon Ishikawa. And so our quest to sell and submit our
BedBusters Housing Fix Bundle continues.

Keith “Buster” Torkelson” has just attended the 1p-4p interval associated with the OCHCA MHSA Steering Committee Meeting (SCM). Buster is Mentalation Solutions Group’s (MSG’s) principle. This email is concerns MHSA funded housing fixes funded with MHSA INN monies. We call our potential solution BedBusters. From the SCM we collected information on current implementation of MHSA Innovations projects update. We are at a loss of how to submit our BedBusters INN project idea. It could be funded as a Prevention and Early Intervention (PEI) program. Since the last time we submitted an idea the process has substantially changed. We’ve attempted several times to connect with an HCA resource to get us on track In due course: During the Zoom SCM Meeting stakeholders commented on BHS Technological Needs. Keith “Buster” Torkelson MS worked on Kathleen Murray’s Team (KMT) for about four (4) years. During that time the team with MSG worked several projects. We were the OCHCA BHS MHSA Technology Advisory Committee or TAC. Projects we the TAC team worked on and closed were: HCA BHS’s Electronic Health Record, Personal Health Records, Tele-psychiatry, E.H.R. Meaningful Use, and Consumer and Family computing with a concentration delivering HCA BHS Clinics Kiosk workstations for clients. Mark Refowitz was actually vested in the Kiosk project. In a TAC meeting he said: “I know you will get this right”.
Consumer Computing and Help@Hand
The Technology Advisory Committee (TAC) disbanded in the middle of Consumer Computing. Buster was the consumer liaison between the community and the Kathleen’s TAC. Kathleen wore many hats yet she identified most as the IRIS Liaison. Kathleen has now retired. The county as well and the community will miss her and her talent. During the 2012 cycle of Innovation Project Ideas (IPIs) a consumer/family technology fix idea was brought up to vet by the Innovations Advisory Committee (IAC). Unfortunately it was not selected to move forward into the planning stages. FYI - Abandoned MHSA INN project see figure below. The project title was: Access to Mobile/Cellular/Internet devices in improving Quality of Life (QOL). At the time was considered a “Good Idea”. We here at Mentalation Solutions Group (MSG) are in the process of submitting a technology-driven housing fix bundle. Again, we call it BedBusters. A total of 22 Innovation Project Ideas were “vetted” in the 2012 cycle. Project 14 was designed to building a consumer technology infrastructure that would enable all consumers the resource and opportunity for equal enagement with more complex technology solutions such as Psychiatric Advanced Directives (PADs).

Help place a BND client Second Try
Buster’s new and improved Brand New Day (BND) helper has offered Buster two chances to place a BND consumer and client into a Rent A Shared Room (RASR) environment. Even with Buster’s fairly extensive RASR housing resources he was unable to land the last client in a GoodBed (MSG). Buster made phone calls and site visits just to discover the RASR options are in more disarray than ever. Two of the bed owners indicated that Corona has caused them to alter their services and products (bed and house).

Catch up with conference lessons: Consortium of Universities for Global Health (CUGH) 12th Annual Conference: Addressing Critical Gaps in Global Health and Development. March 12, 2021 – March 14, 2021. Apply principles locally in our efforts to close “critical gaps” about housing the disadvantaged including the homeless. We feel that housing, rest, and sleep social and medical determinants for Health. People should have the right to sleep in a health-promoting environment. If “Buster” were back in Davis California he would visit upon some of the One Health-ers.

Back in 2014 Buster and Tork Reconstruction Company (TRC) were publishing their Accountability Newsletter. It made sense that they could help Buster’s insurer Brand New Day create a quarterly consumer driven newsletter. Unlike BND existing newsletter, Buster’s newsletter featured real people and promoted assessment. He got a bit of help from his BND helpers yet when it came time for approval to distribute BND dropped the ball. Buster and TRC called the newsletter “Better Than Ever, Incredible Health”. Since they had done most of the work Buster decided to share it in one of his Blogs.

First Question
Is Keith “Buster” Torkelson MS: Routinely
Hungry or Malnourished? For all practical
purposes he is not overtly hungry or malnourished. Yet, Buster has some unmet specific appetites
such as steak and eggs once a week. He
is a bit malnourished in that if he had more food money he would probably
heathier foods. During this first year
of Corona (COVID-19) Buster has been asked several times for donations to feed
America’s hungry. He also has been asked
for handouts by homeless people for what they say is purchasing food. Even though we promote World Wafers to
starving peoples in other countries World Wafers applies to us at home as
well. One of our mantras in the Mentalation
Solutions Group (MSG) World Wafer Division (WWD): “Feed a hungry planet”. The hungry numbers indicates that the US is
doing pretty well. Tell that to a hungry
person! Make note that
epidemics/pandemics go hand in hand with famine and war. We promote World Wafers to help divert the
fallout in other high risk areas such as regional or global disease and
war. This is still the assignment to
satisfy Dr. Calvin Schwabe’s request of his students back in the late
1980s. Dr. Schwabe taught the graduate
course: World Food Problem. One of his
requests was: “Think global, act local”.
Eventually we will publish our World Wafer notions in a standalone
report.
