Executive Summary – Strengths About Chester D Mojica – Abstract
The material for this report was collected over the course of the last four to five years.  This study focused on Doctor Chester Diokno Mojica MD.  Doctor Mojica is in his fifties and thus if encouraged may have time show measurable improvement.  We start with objective materials offered online by his clients.  Overall his clients give him an average of 3.6 Stars (20170521-SUN) out of 5.0 Stars.  We go on to ferret out some features about Doctor Mojica including his strengths.  To focus on his strengths is an achievement for us because he really on focusses on our deficits.  The motivation for our more thorough investigation of Doctor Mojica is that we may together achieve measurably better quality service and more satisfying transactions.  For serving us Doctor Mojica is associated with Brand New Day (BND) a program rated by CMS at 3.0 Stars out of 5.0 Stars.  It is possible that as he improves his quantitatively measured performance the overall program may show improvement as well.  AVEY and several others reviewed this body of work for Mr. Torkelson.  We selected about six (6) Assessment (reserve word) Tools out of over fifteen (15) for this study.  We start by focusing on our subject, Doctor Chester D Mojica’s Strengths (reserve word).  We offer up objective evaluation from two online sites: Vitals and Healthgrades.: We repeat the same objective material in a graphic style.  Near the end we compare quality assurance programs across multiple doctors and multiple industries.  Finally in the appendix we share the invoice for this publication.  There are a couple areas we address yet are not listed here in the Abstract section.

A Word on Satisfaction Assessments
We here at MSG are just going through the paces with most of the assessment tools used for this study.  Over half of them have not been validated or tested for reliability.  Yet, as time passes we have discovered that even the health and human service professionals are writing assessments as needed (PRN).  We didn’t use one of our favorite Satisfaction Assessments, the Hewlett Packard (HP) Support Experience Survey (SES) instrument here. Yet the HP Tool has most likely achieved a stellar level of use (penetration).  We here at MSG submit to HP at least one SES per year.  We will discuss the HP instrument later.
 
Data Rich
This study is data rich.  We remember when it would suffice just to collect one or two data sets for a University of California assignment or other project.  Each of the five (5) or so assessments (quantitative) listed above yield one or more data points.

Formalizing the paper and publishing it online is our new strategy for an alternative to just noting Annoyances (AnnoyMan) using our CaseNotes Utility (MSG).  We now will begin sharing our lived experiences (LiveX) with Dr. Mojica online in our new and improved Blog.  We just wish to share that our Dosage (number of appointments) in “The Clinic” is in par with our needs.  We are truly grateful for the care Doctor Mojica has afforded us (Circa 2013 to 2016).

Strengths Up Front (SUF) – Commentary
Unfortunately Doctor Chester D Mojica (CDM) our Primary Care Physician (PCP) over emphasizes our deficits.  Corporate, the one we believes cuts part of his pay check, wrote us a letter that said we did everything they needed from us in 2016: Keep up the good work.  In addition: Brand New Day (BND) Corporate also subsidized us with a box of gifts as a reward for our “perfect” performance.  While practicing External Quality Review (EQR) we focused on our efforts about high performing human health helpers.  As time progressed we addressed our underperforming health helpers yet we ciphered their identities.  This report is landmark in that we are entering a moderate performer un-ciphered in the digital record.  We had planned just to call Dr. Mojica Bozo-the-Doctor (BTD).  We feel he doesn’t qualify for a BTD rating anymore.

* - Interpretation
3.5/5.0 and 3.6/5.0 Stars would both equate to 2.9 GPA or B- Performer.  This is why we had hoped to see that CDM has published his CV or curriculum vitae.  You may not know that many medical schools accept C or better work as passing.

** - We have been trying to figure out how many Brand New Day (BND) consumers contributed to their 2017 The Centers for Medicare & Medicaid Services (CMS) Stars Score.  We assume that it is greater than one hundred and thus statistically significant.  It looks like they are going to include a Consumer Behavioral Health Experience Satisfaction Assessment (CBH-ESA) in addition to HOS – CAHPS - HEDIS for calculating Stars.

Helping 2012-2017 – Break Even
Basically MSG’s relationship with Chester Diokno Mojica MD is at a break even.  In other words he has helped us about as much as he has frustrated and disappointed us.  We will reduce our comparing him with John Hanley our family doctor.  Our family doctor did it all well including surgery.  The only time we didn’t rely on him (1961-1982) was for a broken arm.  In hindsight he did have one shortfall and that was his prowess at Behavioral Health.
 
What is a Mediocre PCP?
Basically, a mediocre PCP is below average and not very good.  We will flesh out and Vet (reserve word) about mediocrity just a bit for now.  Mediocrity is associated with a “D” grade.  D performance would have to develop sometime after a doctor graduates from medical school and passes the medical board exam.  For medicine we propose a higher standard: In general average is performing at a “C” level.  We here at MSG hold doctors more accountable by proposing average work be at a “B+” level.  In California the minimum performance standard for Graduate School is a solid B.
 
LiveX (2012) - How Much Can a Mediocre MD cost?
In 2012 a mediocre and digitally invisible doctor cost us here at MSG between $15,000 and $20,000 in unnecessary medical fees over the course of twenty days or so (May-June).

Compare with RealAge Method
More than three years ago we tested the online Sharecare RealAge (Doctor OZ) health data engine.  There are many promising features to Sharecare’s RealAge assessment system.  Since Doctor Mojica has no data collection platform for consumers he could save a lot of time and money promoting his clients use the Doctor Oz associated RealAge system.  For the life us we cannot find our Baseline Data (Circa 2009).  News flash we did a paper archive search last Wednesday May 24, 2017 and we stumbled on our results!  We will share our RealAge numbers with our next paper.  After skipping a year of RealAging It we gave the data engine a try for 2017.  An important aspect to share was that with our qualitative interpretation report ShareCare balanced deficits with strengths.  For example, they use statements such as: Keep doing this, you do this right, and work on these items.

Change Gears - Collocation of Services
Here we will offer up a few words based on lived experience (LiveX) on the colocation of services.  Colocation for services such as Primary Care (PC), Behavioral Health (BH), and Substance Use Disorder (SUD) may be a Promising Practice (reserve word).  From approximately 2000 to 2007 Kaiser Permanente served us.  At both their Santa Ana and Aliso Viejo Offices they co-located PC and BH.  Brand New Day co-locates as a minimum their PC, BH, and SUD services in their Garden Grove Facilities.  An issue with colocation is that if say a PC contractor is underperforming it is very hard to replace them at the contractor level.  Substitutions will have to be made in-house or within the contractor organization.

We will address MSG’s Beta Version HEDIS Fitness Assessment (HFA) in a subsequent paper.  As we stated earlier Brand New Day (BND) Corporate indicates that we are doing very well.  Doctor Mojica failed to point out any of our strengths during our last year (2016) physical follow-up appointment.  We are really proud of some of our health related achievements such as attending our BND subsidized Yoga Class a well as our gradual loss of excess weight.  In our lifetime we peaked at 200 pounds.  We now are in the 160s :o).

We surfed the Internet to find indicators for Doctor Mojica’s digital Penetration.  We find that he is relatively underrepresented.  For our next appointment if he were game we would show him the images associated with our Digital Appointment utility.  Once again our primary goal is to make the most of our appointment.  Our second goal is to digitally preserve our Lived Experience (LiveX).  Our prefix assignment for images included in this study is: Keith Torkelson and Chester D Mojica.  We evaluated and selected about 35 images. As two last steps after a cool down period we will label the Images and improve the formatting.

Change Gears - Electronic Health Record (E.H.R.) SuperUser
How time flies!  About three or more years ago we put in a request to do an E.H.R. SuperUser gig with the Orange County Health Care Agency (OCHCA).  The OCHCA runs a Cerner E.H.R. build. Their E.H.R. is run on county owned and maintained server(s) rather than cloud based.  Due to Conflicts of Interest (reserve term) we were not approved to surf the OCHCA system.  The reason we mention this tidbit is small yet important to the consumer things like birthday wishes can be generated automatically via the E.H.R. if set up correctly.  An example was how our dentist sent us a birthday reminder this year (2017).  Dr. Mojica is behind the curve for leveraging the functionality in his (cloud based) E.H.R. system.  

Detour – SuperUser (Barbara Drew) – Patient Portals
Back in 2011 we here at MSG attended one of the nations most prominent Health Technology Conferences.  We were educated by a gal named Barbara Drew out of Napa County California.  She presented on an Electronic Health Record (E.H.R.) Tech Solution she called a SuperUser.  At the end of the presentation she said if you have any questions contact her.  We did.  On 2011_04_22 we received her “job description” about her self defined job description for the SuperUser.  We here at MSG fulfill the majority of the qualities for a SuperUser.  It is unlikely that we will get permission to surf Doctor Mojica’s E.H.R. system.  In an earlier report on Brand New Day’s (BND’s) patient portal we did recommend that a portal rather than being populated separate best be a read only version of the providers E.H.R. (Transparency – reserve word).

We here at MSG have worked very hard on developing our objectivity.  Eventually we hope that the doctors evaluate (Accountability reserve word) themselves.  In addition, we hope that more and more doctors’ share (Transparency reserve word) about themselves and their performance.

Cutoff – Five Star Performance
Our cutoff for mediocre is 51%.  So by our 51% standard, doctor CDM would not be considered mediocre.  Doctor ASK in her thirties seems to be way more promising than is CDM in his fifties.  Doctor John Hanley was our - I can do it all - family doctor circa 1963 to 1977.  Doctor Hanley set the bar high for Primary Care Physicians (PCP) and was anything but mediocre.  From lived experience (LiveX) mediocrity can cost the consumer unnecessary money.  Mediocrity can lead to other losses including injury to the client.  Recall that it was our objective raters that gave Doctor Mojica on average 3.5 Stars (MSG Standard - Mediocre) of 5.0 Stars.  A reason that doctor Mojica is tolerated is that one of the programs (Brand New Day) that retains him scores only a 3.0 (CMS Stars) out of 5.0.  This offers CDM a temporary state of invisibility.  We have discussed Brand New Day’s (BND) CMS Stars scores in other publications.  We have found some in the BND network as performing above 4.0 stars.  We recommended Brand New Day discovers and addresses their 2.0 performers first.  Thus, for the time being doctor Mojica with his 3.5 Stars will remain invisible.

Trust and Confidence - Interpretation & Translation
Would we here at MSG trust CDM with something serious such as a cough that interferes with sleep? – At this time not really.  Example – The Coincidental Antibiotic Story - We are going to save over our vignette about doctor Mojica and our request for an antibiotic.  Would we have trusted him with an intervention requiring fine motor skills such as our cataract surgery? – At this time he has not demonstrated his fine motor skill proficiency – So not really.

Old Car – Harm Reduction
Our principal Developing And Growing (DAG) is 57 years old.  He is like a 60-year-old car.  You don’t have to fix everything – just enough to keep him up for running and satisfying his gal.

Brand New Day issues quite a bit of paper on how to do it.  Most of the documentation comes because they are required to do so.  An example would be the yearly Formulary (reserve word) of covered medications.  Right off in 2012 we were certain that before we had to write our own Do In Manual about the CDM service spectrum that he would give us one.  For example a yearly physical checklist would come in handy.  The Do It Manual might also include a Satisfaction survey template.  Possibly the most important would be the informed consent material for let us services such as HIV Testing or the risks surrounding Influenza Vaccines.

Appendix - Peeve of the Year (Summer 2016)

We are going to wind down by addressing our biggest peeve for 2016 about Doctor Mojica.  We are still trying to decide if this peeve is worth addressing now because it is very subjective.  Yet, it is a good opportunity to presell our Subjective Report in the making.

Medication Change – Wasting Medicine – Lipid Vignette
This year we were prescribed by Doctor Mojica a medication change.  He bullied us.  We ask: In what way will upgrading our Cholesterol control medication improve our performance?  It's not the change of medication that inspired us to write this Lipid vignette as much as it was the process.  We will address this process in a standalone report.  For now suffice to say: We get the new medicine via courier from the pharmacy with a note stating cease using the old medicine.  We can see that wasting a thirty day supply of medicine is in order if the medicine is harmful.  What happened to finish up what you have then begin the new medication.  Think of how much money it costs to unnecessarily throw out medication in a transition.  We will try to get a photograph of at least one person’s medication cache.  Some odd 90% of the inventory is abandoned medications.

LiveX – Kaiser PCP – Safety (Compatibility) Report
Years ago when a Kaiser PCP was going to put us on an antibiotic he had an App which he entered my some on five medications to generate a safety and tolerability report.  We just know that doctor Mojica entered Atorvastatin in his E.H.R. and it generated a safety report.  We will check out our patient portal to see if his safety report has been posted.

Old Medicine – Lovastatin – Experimental Bunny Rabbit (EBR)
Doctor Mojica stated that our 2015 labs indicated that Lovastatin was working.  This year he changed it to Atorvastatin (“something stronger”).  Well we guess the Lovastatin was just an experiment.

Again we used CMS Stars and on-line Rate Your Doctor (RYD) such as Vitals and Healthgrades as our controls.  Unfortunately, for the doctors we have been assessing the total number of reviewers for each is less than one hundred (100) individuals.  Of our CDM associated doctor cohort Doctor Boyce (n=25) has the most reviews.  So few reviewer submissions at this time benefits the doctor(s) because if they promote strong reviews their scores can be significantly improved.  For the time being the RYD scores are just a benchmark.

We here at MSG try to compare like elements as much at possible.  We compare PCP’s and family doctors with each other. We compare specialists with specialists.  We compare psychiatric specialists with other psychiatric specialists – etc.  When it comes to comparing environments such as waiting room and waiting process all doctor’s offices are fair game.

During our first year of service and orientation about Dr. Mojica we were very sick, run down, vulnerable, and fatigued.  Doctor implied that we were not doing it right.  As Doctor Mojica said we were not doing it “how we do it around here”. So to get it right and improve our Prosumption (MSG) we prepare between appointments. Our goal is always for minimizing the teams stress while waiting for, during, and after the appointment.

How is it that we prepare for an appointment?  As a Prosumer (MSG) we prepare so we may maximize positive service outcomes.  We try to quantify as many aspects as possible.

For us here at MSG to capture the Primary Care Physician (PCP) encounter experience we are developing a standard form set.  We use standard forms in determining more precisely what went well in an appointment.
 
Pre-sell for Lipid Report - MyQuest Lab Results (Lipids Plus)
We here at MSG suffer some trust issues.  Sometimes we believe others are not being trustworthy.  We will address some findings from our BSA virtues (and values) assessment in our Subjective Report.  On: 20170403-M: We received some 2015 lab results from Quest Diagnostics.  We had to submit our request via the paper route because Quest’s electronic route only supports automation back to 2016.  Quest sent us via US mail the wrong results.  We received three months worth of CBCs while we requested our 2015 Lipid Panel results.  Eventually we sent enough information that spelled out our concerns.  On round two (2) Quest sent us the correct results.  How is it that we want our 2015 Lipid results?  We will answer this in our Lipid Report.

Hardware and Software Supporting Assessment
Back in 2009 we began our association with the Orange County Health Care Agency (OCHCA).  We were assigned work in par with Technological Needs about Health and Human Services.  The smallest project using Mental Health Services Act (MHSA) monies was the Kiosk Project.  We delivered Kiosks to more than five (5) county clinic locations.  Central City Community Health Center (CCCHC) might best invest in resilient and durable Kiosks for the waiting room.  As we discussed earlier CCCHC at this time (May 21, 2017) is not offering Satisfaction Surveys.  At this point the clients would have to submit paper satisfaction and other surveys such as a Personal Health Questionnaire 9 Item (PHQ-9).  If one or more Kiosks were available, running some custom Apps, data collection such as Satisfaction would be streamlined. 

Goal To Close Atorvastatin Annoyance
How this study got started was that we here at MSG needed a way to process what we were exposed to in a typical appointment with Doctor Mojica and Staff.  In preparing to tolerate our next appointment our notes (CaseNotes) and concerns (ConMan) became pages and pages became a report.  FYI – Our next appointment is scheduled for – 20170706-TH @9am.  We will be determining for us the efficacy of Lipitor's generic atorvastatin.  We will share more in another article: Doctor Mojica and Lipid control efforts.

What’s Next - Proof of Concept for Quitting Smoking
We here at MSG smoke cigarettes.  The new California per pack cigarette tax has given us a reality check.  We had hoped that the tax would support proven strategies to help others and us quit smoking.  We will address Doctor Mojica Smoking Termination program at a later date.
