Abstract – Executive Summary

This report pertains primarily to Keith Torkelson, MS and his partnership with Nurse “Martha”. Martha is the nurse lead for Central City Community Health Center (CCCHC) in Garden Grove California. We are Mentalation Solutions Group (MSG) and we engage in Advocacy and External Quality Review efforts about Health & Human Services. Nurse Martha G Gil (Martha) first served Keith during the summer of 2012. Keith has had more than fifty (50) encounters with Nurse Martha since this time. Our partnership is concentrated about her role as a phlebotomist for Keith’s monthly Complete Blood Count (CBC) lab work. Martha has never “blown out” any of Keith’s veins. We here at MSG have a standardized set of measures we use for evaluating Keith’s clinical (and clerical) relationships with his health providers. What follows are our findings for year 2018 centered about Nurse Martha.

Vital Plus and Quick Scoring (Service Interval 2018)

While applying our Vitals Plus Assessment Tool we discovered that Nurse Martha delivered a 4.6 of 5.0 stars Level Of Service (LOS). We differentiate our MSG "stars" from The Centers for Medicare & Medicaid Services (CMS) "Stars" by not using Title Case, as does CMS. In addition we call our stars Star Equivalents (SEs). Martha's team earned 4.0 stars for clinical and clerical help. On and off we add a bonus point for improvements that we the partnership have sustained. With a bonus point we give Martha 5.0 (5.4) stars for an “A+”. Martha is what we call the team lead nurse for Central City Community Health Clinic (CCCHC). We included objective information about CCCHC that we found recently on the website “Yelp”. On 20181203-M six (6) “Yelpers” had reviewed CCCHC. CCCHC (Garden Grove) overall Yelp rating is 1.0 star(s). It would appear that Martha is a very high performer about the CCCHC operation. Hats Off! To Martha G Gil. In our online Yelp review we gave the whole agency (CCCHC) Martha’s score of 5.0 starts even though overall they deserve a score somewhere in the ballpark of 3.0 stars. We earned no freebees for sharing this evaluation. (AVEY)

We take the CCCHC data point of 1.0 directly from Yelp online. The CCCHC data point was generated for the Garden Grove Clinic from six reviewers. Most of the people we deal with at CCCHC score in the Blue Zone. We consider a deal as talking with them one-to-one, having them take a measure such as weight, and working one-on-one in an exam room. We propose that the individuals that operate at or near the Red-Yellow boundary are individuals that have a higher impact on Yelp consumer reporters. Years ago they CCCHC replaced “Steve” at the front counter. We found it to be an improvement. It is unlikely that CCCHC will replace whoever is in the Red Zone.

Primary Engagement with Nurse Martha

We enjoy leveraging our patient portal into Quest Diagnostics records. We will not address the Clozapine CBC requirement at this time. Team Martha and more often that not Martha herself draws our blood every 28 days for our CBC lab work. Quest Diagnostics is the lab that performs our CBC testing. See the Green dot on the far right. It is very likely it is good news for Buster AKA Keith. (AVEY)
